Product Advisories: Bulletin 1494V-DS200/B and 1494V-DS100/A Disconnect Switch

Affected Products: Products includes all 1494V-DS100/A and -DS200/B disconnects manufactured prior to July 2000.

Symptoms: In certain Bulletin 1494V disconnects, the bolts holding the operating mechanism to the switch mechanism may become loose. If this occurs, the operating mechanism may become dislodged from the switch in a way that makes the operating handle indicate that the switch is "off" when the switch mechanism is in the "on" position.

Corrective Actions: Any unopened shipping cartons in inventory may be exchanged via the Return Authorization procedure (RMA). Installed modules may be replaced on an exchange basis via SMS Web Access. RA also recommends that kit (CAT# 1494V-H13) be installed to ensure that bolts remain secure. This kit can be ordered at no charge from Passport.
