Product Advisories: Bulletin 1494V Series 2 Variable Depth Disconnect Switch Kits 30A, 60A and 100A

Rockwell Automation has determined that some Bulletin1494V Series 2 Variable Depth Disconnect Switch Kits may malfunction and create a serious safety hazard that could result in serious injury or death if not installed properly. The new Series 2 disconnect switch kits launched on March 14, 2005 have different mounting hole locations as compared to previous 1494V Series 1 or Series A Switch kits. If the disconnect switch is not installed properly, the end user may have difficulty energizing and de-energizing the switch thus creating an unsafe condition. Therefore, it is imperative that the new instruction sheet be read and understood before product installation.

Problem Description

The new 1494V series 2 Variable Depth Disconnect Switch Kits have external mounting holes that are different from 1494V Series 1 or Series A Switch Kits.

The new 1494V series 2 Variable Depth Disconnect Switch is positioned higher in the panel with respect to the connecting rod and handle. The connecting rod has a slight upward angle, and is not perpendicular to the enclosure flange like the 1494V Series 1 or Series A Switch kits. If the disconnect switch and connecting rod are not installed properly, the switch could remain in the energized position (ON) when the handle is moved to the (OFF) position thus creating an unsafe condition.

Problem Correction

A product information tag has been attached to one of the mounting holes on the switch of the 1494V Series 2 Kit, advising the user to read and understand the instructions before installation. The product information tag should be removed in order for the switch to be installed on the mounting plate. The layout of the instruction sheet (Publication 42052-116-01(2)) has also been modified.

NOTE: When printing the applicable templates, they must be printed on 11” X 17” size paper. The template dimension is critical for proper installation.

Requested Customer Action

  • Please read this Product Safety Alert and perform the following action immediately.
  • Please read the instructions (Publication 42052-116-01(2)) completely before installing any 1494V Variable Depth Disconnect Switches. NOTE: The applicable templates must be printed on 11” X 17” size paper. The template dimension is critical for proper installation.
  • Please inspect any variable depth disconnect switches already installed and verify that the switch is properly adjusted, (per the instructions along with the switch and handle templates), so it is de-energized when the handle is moved to the OFF position.
